twice, both times were when my son got sick...the first time he was a 10 month old baby and he got a really really high fever and i had to take him to the emergency room in the middle of the night...the second time he was around 8 years old and we thought he had appendicitis but after lots and lots of tests it turned out okay.
peace...

The most nervous I've ever been is right before giving birth (all three times). I ended up having a c-section with all of them, and there is a point right after they give me the spinal and before they let my husband into the room where I feel like I can't breathe, then I start to imagine everything that can go wrong and I have a complete panic attack on the surgical table. This last time I had a wonderful nurse and the best anesthesiologist who helped literally held my hands and talked to me and helped calm me down. Oh, and each birth turns out just fine... really easy and the babies have all been perfect. But I still have a freak out point.

A few years ago, I was out at a meeting, and came home to find my wife passed out on the floor. I rushed her to the hospital, literally driving about 60 in a 25 MPH zone. Turns out she had an allergic reaction to some medicine she was taking.
At the time, I wasn't sure if I was going to lose her or not.

the first thing that comes to mind is my back surgery. i was 12 and had to go into a 4-6 hour surgery where they were going to try and correct my scoliosis, fuse some vertebrae together, and stick a titanium rod in my back. there was lots of risk involved, and the dr and my parents were both upfront about this. i was terrified.
